Phase 2 wires consultation auto-assignment: when a screening row is promoted to a consultation, a 3-step attending fallback resolves the right doctor — department_queues.assigned_to, then the encounter's assigned doctor, then the hoisted attending id. Both phases ship flag-off with zero behavior change. Activation is a data toggle at /admin/auto-assign — the engine stays inert until a hospital opts in.
